In order to have the most accurate reading possible please provide the following information:

-Birth date (MM/DD/YY)

-Exact time of birth (12:01AM)

-Birth city and state (City, State)

*At this time in my practice I do not offer Birth Chart Rectification which is an alternative technique used to determine the rising sign without a birth time. Below are instructions on how to request a long form birth certificate which in most cases will provide the time of birth.

If you don’t have your birth certificate handy, VitalChek.com is the fastest way to request a copy. To ensure your birth time is included, follow these specific steps:

1. Select the "Long Form": When prompted for the type of certificate, always select Long Form, Vault Copy, or Full Image.

2. Choose the Right Reason: If those options don't appear, look at the "Reason for Request" dropdown menu. Choosing "Apostille/Authentication" or "Dual Citizenship" almost always triggers the state to send the Long Form (which includes the birth time).

3. Add a Special Note: In the final comments or "Additional Information" box, explicitly type: "Please provide the full vault copy including the time of birth for genealogical purposes."

Note: Expect to pay the state’s certificate fee (usually $10–$20) plus VitalChek’s processing and shipping fees.
